This Washington My Health My Data Act Policy (“MHMDA Policy”) applies to the collection of consumer health data from Washington State residents. This policy supplements our “Privacy Policy” to provide Washington State residents with information and rights required by the Washington My Health My Data Act (“MHMDA”). This policy applies only to individuals residing in the State of Washington who are considered “consumers” under the MHMDA and from whom we collect “consumer health data” as described in the MHMDA.

What is consumer health data? Consumer health data is personal information that is linked or reasonably linkable to a consumer and that identifies the consumer’s past, present, or future physical or mental health status.

What categories of consumer health data do we collect? We collect the following categories of consumer health data: individual health conditions and bodily functions and symptoms. We collect consumer health data from you directly when you shop our website and voluntarily provide it to us by taking our health quiz. We share your answers to our health quiz with our website provider that provides us with the web page that collects the answers. We use your answers to the quiz to suggest certain product selection choices. To the extent you take our quiz, you consent to the sharing of your consumer health data with our website provider and use of consumer health data to provide you with informational and marketing materials, including product selection choices.

Do we sell your consumer health data? We do not sell your consumer health data.

How to exercise your rights? If you are a resident of Washington, subject to certain exceptions, you have the right to confirm collection of and to obtain access to consumer health data, and to delete or withdraw consent as it relates to Consumer Health Data.
